Several studies have established the prognostic value of vasodilator stress CMR in broad population of patients with suspected or known coronary artery disease (CAD), but this specific population of asymptomatic patients with known CAD have never been formally evaluated. Therefore, this study aimed to assess the long-term prognostic value of vasodilator stress perfusion cardiovascular magnetic resonance (CMR) in asymptomatic patients with obstructive CAD.

What is the current state/problem in practice? The assessment of the prognostic value of vasodilator stress CMR in asymptomatic patients with known CAD have never been formally evaluated. While risk stratification of asymptomatic patients can be useful in managing secondary prevention, the European and American guidelines do not recommend systematic stress testing in the follow-up of patients with CAD, because they are mainly based on studies of symptomatic patients. What is the ideal state/ if the problem is gone? An improved risk stratification using stress CMR in asymptomatic patients with known CAD could allow to identify high-risk patients who could benefit from treatment intensification, new therapy and/or revascularization.

 

Purpose/ Objective

Knowing that in asymptomatic patients with known CAD, the presence of inducible myocardial ischemia and LGE assessed by stress CMR could be helpful to stratify the risk of CV events recurrence.

 

Learning Objectives

  1. Learners need to know about (knowledge): the prognostic value of stress CMR in asymptomatic patients with known CAD
  2. Learners need to know how to apply (intention to put into practice): the interest of the detection of the presence of inducible myocardial ischemia and LGE assessed by stress CMR to stratify the risk of CV events recurrence.
  3. Learners need to do (perform): to analyze images perfusion and LGE to detect the presence of inducible myocardial ischemia and LGE.
